Aalto Home Care was built by people who saw a gap between what home and community-based care should feel like and what it too often does. Our founders bring hands-on experience in care coordination and direct support, and started Aalto Home Care to close that gap – with a team that communicates clearly, shows up reliably, and learns each person’s routine, language, and family, and keeps them in the loop.
To deliver compassionate, dependable, and culturally-responsive home and community-based care that helps individuals live safely, independently, and with dignity.
To be a community-based care provider that families and referral partners trust across the Twin Cities, with personalized support that improves quality of life.
Aalto Home Care is a 245D support provider. We are not a nursing service. Our approach draws on Jean Watson’s theory of human caring, which shapes how we care. It does not change what we deliver.
Each visit is a chance to connect, not just finish tasks.
Staff get to know the person over time.
We pay attention to routine, culture, faith, and goals.
A calm, respectful, safe space at home.
We ask what matters to the person and plan support around it.
Everything we do is grounded in what we call C.R.E.A.T.E. with Love. These aren’t words on a wall; they’re how we hire, train, and hold our team accountable every day.
Quotes from people we support, shared with their permission.

“They always look out for me. My apartment changed my whole situation, and the staff never fail to make sure I’m okay.”
“I got my own space, staff check in on me daily, and I get help with everyday errands. I’d totally recommend them to anyone who needs it.”
